First Communion Form

1. A Baptism Certificate must be Presented to church with the registration;

2. Both candidate & immidiate family must participate in weekly Sunday Masses at Our Lady of Lebanon and in the parish Activities

3. Both Parents and candidates must volunteer at least 15 hours in church Activities and events (ex. Festival...)
